All returned material services require an RMA. To acquire an RMA number, please fill out the RMA Request Form below or contact FiberZone Customer Support to obtain a request form. FiberZone Customer Support will review the request form, and contact the requestor if additional information is required. Once the request is complete, the FiberZone Customer Support team will issue an RMA number via email.
Upon receipt of the RMA number, the product may be returned to the following address. Please ensure that the RMA number is clearly visible on the outside of the shipping package:

Note: FiberZone Networks will only accept material with a valid RMA number on the shipping package. Any material received without a valid RMA number will be returned un-serviced to the customer at the customer's expense.
